2026 BCCHF All-Banks Slo-Pitch Classic
Release, Waiver & Assumption of Risk Agreement – For June 12, 2026
READ THE FOLLOWING SECTION FULLY AND CAREFULLY PRIOR TO SIGNING. THIS IS A LEGALLY VALID AND BINDING OBLIGATION TO RELEASE THE BANK OF NOVA SCOTIA, THE CANADIAN IMPERIAL BANK OF COMMERCE, THE ROYAL BANK OF CANADA & TORONTO-DOMINION BANK (the “Company”) AND BC CHILDREN’S HOSPITAL FOUNDATION FROM ALL KNOWN AND UNKNOWN OBLIGATIONS, AND TO INDEMNIFY
THE COMPANY AGAINST CLAIMS BROUGHT AGAINST IT BASED UPON YOUR ATTENDANCE OR ARTICIPATION IN ANY WAY IN ANY SLO PITCH TOURNAMENT OR OTHER TOURNAMENT OR EVENT AT SQUINT LAKE PARK IN BURNABY, BC (hereinafter called “SL”) THIS INSTRUMENT AFFECTS IMPORTANT LEGAL RIGHTS.
In consideration of me being allowed to attend at or participate in any way in any slo-pitch tournament or other tournament or event at SL (the “Activities”), I warrant that I am physically fit to participate and of legal age and competent to sign and agree that a benefit is realized by me through the Activities and I hereby for myself, my heirs, executors, administrators, successors and assigns release from liability and agree to indemnify and hold blameless the Company, the promoters, all other participants, officials, sponsors, advertisers, league executive, owners and/or operators of the premises and facilities and each of them and any of their agents, employees or representatives or otherwise, during my participation in the Activities. In addition, I agree to, at all times, inspect equipment, playing fields, premises and event-related facilities for possible risk and determine for myself that conditions are acceptable for me to commence or continue participation and/or attendance. I also agree my participation and/or attendance, commencing or continuing, indicates acceptance of equipment, fields, premises, et al, and I knowingly and freely assume all risks and full responsibility for my participation and/or attendance, commencing or continuing and I hereby grant the Company and their agents the irrevocable right to use my name, face or other likeness for any and all purposes without payment. I am fully aware that the Company carries no medical or liability insurance for any participants or attendees and that I am solely responsible for securing my own insurance.
By signing this document, I intend to forever waive my right to maintain any lawsuit or action against the Company based on any claim of personal injury or death or property loss or damage or neglect whatsoever. I have had sufficient opportunity to read and understand this agreement and consult with legal counsel or have voluntarily waived my right to do so. I knowingly and voluntarily agree to be bound by all terms and conditions set forth herein. I also agree that if any portion of this agreement is found to be void or unenforceable, the remaining portion shall remain in full force and effect.
